During your eye exam the optometrist is not only evaluating your eyes for visual correction (if you need eye glasses or contact lenses), they are also checking the overall health and function of your eye(s).
Most eye diseases have very little or slow symptoms and cannot be detected unless you have a comprehensive eye exam. It is important to detects eye diseases as early as possible as some diseases, like Glaucoma, are not reversible, but they can be managed and possibly prevented.
Examples of what the doctor is looking for:
Refractive error: This refers to your prescription and is corrected with eye glasses, contact lenses or surgery.
Strabismus (eye turn): This has to do with the muscular functions of your eye(s).
Ocular signs/symptoms: Complaints of burning, itching, red, irritated or watery eyes are all common symptoms that can be caused by dry eyes, allergy or digital eye strain.
Eye Diseases/Conditions: These can be detected by looking at the eye’s blood vessels, retina (macula and optic nerver) and so forth. Diseases include Glaucoma, Cancer, Cataracts, Retinitis Pigmentosa, Diabetic retinopathy, Age-related macular degeneration and many others.
Pediatric Eye Exams
The importance of an eye exam for children and infants Since many eye problems arise from conditions that can be identified by an optometrist in an infant’s first year of life, it is highly recommend a child have an eye exam starting at 6 months of age. The earlier a vision issue is detected in a child, the better are the effectiveness of treatment and options available.
Many serious eye conditions do not have obvious symptoms and some eye diseases only show symptoms when the condition is advanced and difficult to treat. Conditions such as amblyopia or strabismus need to be addressed when a child is young to have an effective treatment outcome. The earlier these conditions can be treated, the higher the success rate for best visual outcome.
How Often Should I Get An Eye Exam?
Children: An assessment between six and 12 months of age is recommended to determine if an infant is at risk for eye or vision disorders. Children should at least have had one eye examination by the age of three, and then every year after that (more often if recommended by your optometrist).
Adults: We recommended an eye exam every two years, especially after the age of 40 as the risk of many eye diseases increase with age. An annual eye exam is recommended if there is a family history of eye diseases such as glaucoma or age-related macular degeneration.
Seniors: Annual eye exams are recommended over the age of 65.
What is Diabetic Retinopathy?
Diabetes is a chronic medical condition that affects how the body processes blood sugar (glucose). It occurs when the pancreas does not secrete enough insulin or the body is unable to process it properly. Insulin is the hormone that regulates the level of sugar (glucose) in the blood. Diabetes can affect children and adults and is also a genetic condition.
How does diabetes affect the retina?
Patients with diabetes are more likely to develop eye problems such as cataracts and glaucoma, but the disease’s affect on the retina is the main threat to vision. Most patients develop diabetic changes in the retina after approximately 20 years. The effect of diabetes on the eye is called diabetic retinopathy.
How often should I get an eye exam if I am diabetic?
Annual eye exams are highly recommended for patients with diabetes. If you have signs of diabetic retinopathy, then a retinal evaluation is recommended every 4 months to ensure optimal vision.
How can I regulate my blood sugar level (BSL)?
Routine physical check-ups and blood work are recommended with your family physician every 3 months to keep a close eye on your A1C level (a blood test that measures your average blood sugar level over 2-3 months). Diet control, exercise, and lifestyle changes are also highly recommended to keep your blood sugar level under control. Medications may help to better regulate your BSL if you have done all the necessary lifestyle changes and you still cannot get your BSL to go below or reach the target A1C level which is generally less than 6.5%.
Ocular Surgery
LASIK and Laser Refractive Surgery Co-management
Most people with vision correction issues want to know more about LASIK and whether it is right for them.
During the procedure, a flap is surgically cut in the cornea and gently folded back. An Excimer Laser is used to reshape the cornea into a flatter shape (this is much like a contact lens corrects vision by forming a new shape on the cornea). The flap is then put back in place, acting like a natural bandage. The healing process is quick and the discomfort level is quite low.
People will often say that they did not feel anything, and could see well the very first day. Most people have improved vision in 24-28 hours, but it must be noted that each person will heal at a different rate. Vision, although greatly improved immediately after surgery, often continues to improve for some weeks, even months.
LASIK is not for everyone! Eye conditions vary and only after a comprehensive examination will we be able to decide if you are an ideal candidate for laser eye surgery. Cataract Surgery
Cataract surgery, also known as lens replacement surgery, is the removal of the natural lens of the eye that has become an opaque or cloudy, causing blurry vision. The eye's natural lens is replaced with an artificial intraocular lens implant. Cataracts can either be a slow or rapid progression and is different for everyone. It generally tends to have an earlier onset in life if you are diabetic or taking certain medications.
Symptoms of cataracts include:
Blurry vision.
Fading or yellowing of colors.
Trouble seeing at night.
Sensitivity to light and glare.
Need for brighter light for reading and other activities.
Seeing "halos" around lights.
Frequent changes in eyeglass or contact lens prescription.
If you are experiencing any of these above mentioned symptoms it is highly recommended you see your routine optometrist for a cataract evaluation. Upon your ocular health assessment, if cataract(s) are diagnosed a referral would be made to ophthalmologist for further assessment. The ophthalmologist would perform certain tests that determine which intraocular lens would be best for your visual needs, discuss pricing and would be the one performing surgery. Once surgery is complete, 3-4 weeks following surgery, it is recommended to return to your routine optometrist to reassess your visual acuity in case a glasses prescription is required (this will depend ultimately on the intraocular lens you chose).
